HOW TO REGISTER

  1. Download a registration form (below) or contact the registrar for a camp brochure.
  2. Complete a separate registration form for each child attending camp.
  3. Be sure to read the conditions of enrollment!
  4. Mail your completed registration form with a $100 cheque (payable to Sparrow Lake United Church Camp) for the non-refundable deposit and a post-dated cheque (May 1, 2008) for the balance of the camp fee. If you are paying by VISA or MasterCard please complete and sign the credit card payment section of the registration form.
  5. You will also receive camper health and information forms in the mail after your registration. These forms must be received by the registration office before your child goes to camp.
  6. If you are registering after May 1, 2008 full payment is due upon registration. Camper Forms are due within 2 weeks of receipt of the registration package. Do not mail registration forms within 2 weeks of camp starting. Credit card payments will not be accepted within two days of the camp session starting.

CIT APPLICATIONS

Print out and mail the CIT application.  It must be received by March 1, 2008.

CAMPER FORMS

The following forms are sent out by the registrar upon receipt of your registration forms. If you would like to send these in with your registration form you can. Before completing these forms, check the registration office phone message first to find out if we have spots in the camp that you are interested in. (We wouldn't want you to do all this work and find out a session was full!)

1. CAMP GUIDE: All you need to know about registration, what to pack, bus stop information etc. The clothing order form is in this guide.

2. CAMPER HEALTH AND INFORMATION FORM: This form gives us health, emotional, and social information about your child.

3. SUPPLEMENTAL HEALTH FORM: For LIT and successful CIT candidates only. Must be signed by your family doctor.
